1. First Free Math Individual Tutoring Session, Class, or Chess Class Registration Form

New students are welcome to register. To register for the first free math individual tutoring session, class, or chess class^* please complete the online registration form below or fill out the paper registration form^** and mail it to Columbia Academy. Please contact us for the next available date and time. Enrolment is limited and is on a first-come-first-serve basis. Advance registration is required in order to reserve a place in the class. While our regular small group math tutoring programs (2 - 5 students) provide an excellent combination of individualized instruction with one of our tutors and the benefits of collaborative study with others, a number of students in the first free math class may vary. Once we receive your completed registration form we will email you a confirmation along with more information about your free math or chess class you are registering for.

2. 2012-2013 academic year individual math tutoring, small group math tutoring, academic enrichment programs, classroom programs, test preparation, chess lessons, computer graphic design program, and art classes Application Form.^** Please fill out the form and return it along with the payment to Columbia Academy.

3. Publicity Release Form^**
